On-Line Letter of Inquiry / Application Form

Please read carefully before submitting a Letter of Inquiry/application

Please be sure that you have carefully reviewed the Genevieve and Ward Orsinger Foundation guidelines and application process information from the website prior to starting the online application process.

Please use only plain text. Do not use bulleted lists, bold, underline, or italics. You may use quotation marks, numbered lists, dollar signs, parentheses and hard returns.

You can save the online LOI or application and finish completing it later by clicking on the "Save & Finish Later" button at the bottom of each page. You will be prompted to create an account. You will receive an email confirming your account. Please keep this message and read it carefully. It contains the specific URL you must use to log in and view your saved application. Before you submit the final version, you can click the "Review & Submit" button at the bottom of the last page to read through your application before submitting it.

You should receive a confirmation email containing a copy of your LOI at the email address you provided in creating your account.

In addition to the online LOI/application, please submit the documents requested by mail on the day that you email your application.

Note: INCOMPLETE LOI's/APPLICATIONS WILL NOT BE CONSIDERED FOR FUNDING.
